AAC Now! Practitioner Agreement

 

Intended Use

AAC Now! Practitioner cards provide access and use to CoughDrop and Forbes Premium Boards. The use is intended to allow practitioners to perform AAC assessments and evaluations with potential AAC Communicators. Access to the software is permitted on up to 3 devices. At no time is it permissible to share Forbes Premium Boards, the practitioner card access is only for the registered and assigned user.

Practitioner Statement of Attestation

By registering for AAC Now, you are attesting that you are an AAC professional who performs AAC evaluations and/or AAC implementation. You must provide your current license number and area of specialty to complete your registration.

PHI, HIPPA and Confidentiality

Never share your Username or Password: Your CoughDrop profile may contain Protected Health Information (PHI) that would become accessible to other parties if the credentials were to be shared.

You must treat your username and/or password as confidential and you must not disclose it to any third party. You are responsible for all activities in connection with the Product that occur under your username or password. You agree to immediately notify Forbes AAC of any unauthorized use of your username or password or any other breach of security.

All practitioners must also comply with the Terms of Use outlined on the CoughDrop website.

AAC Now! Communicator Agreement

 

Intended Use

AAC Now! Communicator cards provide access and use to CoughDrop and Forbes AAC’s Premium Boards to Communicators/end users. The use is intended to allow Communicators to access AAC boards while they are evaluating AAC (trials) or awaiting their Speech Generating Device to be funded. At no time is it permissible to share Forbes Premium Boards, the communicator card access is only for the registered and assigned user.

Communicator Trials

Communicators undergoing AAC assessments can trial CoughDrop and Forbes AAC’s premium boards for a 60 day trial period. Upon reaching the trial expiration, Communicators can receive a fully licensed software activation when Forbes AAC receives a completed funding request from the Communicators Speech and Language Pathologist.

AAC Now! Communicator Software Activation

Communicators can receive a fully licensed software activation when Forbes receives a completed funding request from the Communicators Speech and Language Pathologist. To be eligible for activation, the request must be complete and contain all required documents as determined by Forbes AAC.
