Thank you for you interest in our products! After submitting your request via our web portal, you must submit certain documentation based on whether you are an SLP or an end-user. All documentation can be emailed to our sales department: sales@forbesaac.com

Loans for Clinicians
If you are a Speech-Language Pathologist or evaluating clinician, you can request to borrow a device for the purpose of conducting evaluations. Please submit the following documentation to sales@forbesaac.com:
Loans for End Users
If you are an end user who has completed an AAC evaluation and need to complete a trial, you must submit the following required documentation.
Please note: This option is limited to clients for whom funding, such as private insurance or Medicaid, requires an in-home trial. Additionally, an SLP can recommend a trial based on their discretion. Please submit the following documentation to sales@forbesaac.com.
