Request a trial device for an End User
Forbes AAC provides trial devices who have certain requirements from their funding provider. These trials are initiated by licensed clinicians and provided to the client’s family after insurance coverage is verified.
Here’s how it works:
-
Submit a Benefits Check
Start by submitting a Benefits Check Form, available in our Funding Portal. If you don’t yet have access, you can register for the portal at any time to get started. -
Receive a Benefits Check Approval Code
Once coverage is confirmed, you’ll receive a Benefits Check approval code. This code allows you to request a trial device directly through our online Loan Library Webstore. -
Visit the webstore to select your Trial Device
Visit the webstore, choose the appropriate trial equipment, and enter your approval code at checkout to complete the request.
Trial devices are available for end users through a clinician-led process. A licensed provider must complete a Benefits Check, receive an approval code, and submit the request on behalf of the client.
If you're a parent or caregiver interested in a trial, we encourage you to speak with your clinician, who can guide the process on your behalf.
Submit a benefits check to get started
Submit a Benefits Check to confirm your client’s insurance coverage for a speech-generating device. Once approved, you'll receive a code to request a trial device directly through our Loan Library Webstore.
